Refractive surgery has the goal of providing patients with spectacle independence. Refractive surgery first became popular with radial keratotomy (RK). Later, LASIK and PRK became common. The latest generation includes Small Incision Lenticule Extraction (SMILE) and Kerato-Refractive Lenticule Extraction (KLEx).
RK, LASIK, PRK, SMILE, and KLEx all focus on changing the shape of the cornea to improve vision. However, refractive error can also be corrected by changing the power of the lens inside the eye — a more advanced approach as our lens and implantable technology has improved.
This can be done with Refractive Lens Exchange (RLE), also known as Clear Lens Extraction (CLE), which is essentially cataract surgery with a clear lens. Another option is placing an Implantable Collamer Lens (Visian ICL) on top of the natural lens.
Which procedure is right for you depends on your age, eye health, refractive error, and lifestyle. As a general rule of thumb, if you are 45 years or older, the likely best fit is a Refractive Lens Exchange (RLE) or Clear Lens Extraction (CLE).
